Childbirth program forms

Below are the forms you will need to complete and bring with you when you come to the hospital to register for the birth of your baby. When coming to the hospital, please bring the following;

A completed standard pre-admission record;
A completed pre-anaesthetic patient questionnaire;
A completed Initial Maternal-Fetal Assessment Pre-Birth Service form;
Your provincial health card (if you have one);
Your extended health coverage information and policy numbers;
Other papers given to you by your doctor, e.g. antenatal records; and Questions about pregnancy, your birth experience, your hospital stay and community supports.
