Outpatient and community-based programs

Grand River Hospital has a range of mental health services available that do not require an admission to the hospital. For referrals or questions regarding outpatient mental health services please call 519-749-4310 (follow the automated directions to transfer to the adult intake or child/adolescent intake).

Adult services: (19 years of age and older)

Hazelglen outreach mental health service
Hazelglen helps people with mental health problems in the community, instead of the hospital. Hazelglen can be involved quickly to provide intensive, short-term mental health treatment services to help individuals and families get through a crisis. Referrals to Hazelglen are made by the Emergency Crisis Service and by a care treatment team within the Mental Health and Addictions Program.

Rapid response (referrals are made by the emergency crisis service in the GRH emergency department)
A clinician provides a timely assessment, consultation and short term follow-up for individuals. If needed, a consultation with a psychiatrist may be arranged.

Transitional support and treatment
Provides a short-term transitional group treatment program for individuals who are being discharged from any of the hospital’s inpatient or outpatient mental health or addiction services.

Medication follow-up clinic
Provides transitional medication support for adult individuals being discharged from the Inpatient and Hazelglen services. This includes a specialized clinic for individuals on the medication Clozapine. Referrals are made directly by psychiatrists

Community outreach treatment team (COTT)
This team provides treatment, rehabilitation and support to those individuals with a severe and persistent mental illness who have had long or frequent hospitalizations. COTT provides coordination, support and education regarding Community Treatment Orders (CTOs).

Psychiatric consultation clinic
This service offers one-time psychiatric assessment and consultation service provided to Family Physicians to clarify diagnosis and treatment recommendations. Family physician may call or fax a referral.

Services for children and adolescents: (18 years of age and younger)

Child and family
This service offers multidisciplinary assessment, consultation and treatment for children age six through 12 who are experiencing complicated mental health symptoms.

Adolescent team
This service provides multidisciplinary assessment, consultation and treatment for adolescents age 12 to 18 who are experiencing severe or complicated mental health symptoms.

Young adult program (YAP)
This day program is for young people experiencing mental health concerns that interfere with their ability to succeed in a traditional school environment.

Child and adolescent consultation clinic
Referrals from family physicians for a psychiatrist for diagnostic and/or treatment recommendations for children and teens.

Preschool diagnostic and treatment services
Early identification, assessment, consultation and treatment for children between two and five years of age, and their families.
