Privacy and confidentiality

Protecting the privacy of patients is a key responsibility for Grand River Hospital.

Since family and friends of patients are outside of the circle of care, GRH must ensure that express consent from the patient (written or verbal) is received before we share information with these individuals.

The purpose of the privacy code is to facilitate the sharing of personal health information between hospital staff and the family and friends of patients while ensuring that consent directives under Ontario’s Personal Health Information Protection Act (PHIPA) are met.

When you arrive in the medicine program, a nurse will give you a privacy card with the privacy number marked on it. The privacy code is a four-digit number taken from the patient account number. Please use this code for accessing information about a patient.

All account numbers have the format 1-23456-78 and change every visit. The privacy code will represent the last four digits in the middle grouping. For the example above, the privacy code would be 3456 (1-23456-78).

We will mark the privacy code on the privacy code card or privacy pamphlet and give it to the patient or substitute decision maker at the first reasonable opportunity after admission.

We encourage the patient or their substitute decision maker to nominate one spokesperson to be in contact with the hospital regarding the patient’s health status.
