Grand River Hospital provides stroke rehabilitation at our Freeport Site, 3570 King Street East in Kitchener. We provide a range of options for patients who require ongoing care and rehabilitation on an inpatient and outpatient basis.
We work in partnership with patients and their families to determine the best discharge goals and destination. Our goal is to help patients access the care they need, when they need it, and to make these transitions as seamless as possible.
