- Your Ontario health card

- A list of your current medications
- Your insulin/insulin pens if using
- A list of questions you have
We encourage you to bring a family member or support person with you to your appointment.

The clinic is open 8am to 4pm Monday to Friday

We are located on the fifth floor of the Kaufman building at the KW Site of Grand River Hospital. Our street address is 40 Green Street, Kitchener, Ontario.
