Making a request under FIPPA
Grand River Hospital routinely provides information to the public and shares documents with other organization. The Privacy and Access Office welcomes informal inquiries about records.
If you choose to formally submit a request for information under FIPPA, there is a defined process to follow.
Please note that requests received by electronic mail will be acknowledged, however the formal process under FIPPA will only begin once the $5 application fee has been either delivered by hand or mailed to the Privacy and Access Office.

A clearly-defined request will greatly assist the hospital to identify the records/personal information sought. A member of the privacy and access office may contact you about the facts and circumstances of the situation to clarify the access request.

Where to send a FIPPA request
Please send your FIPPA request form and your $5 application fee to the following address:

You may also drop off your request and $5 application fee in person at the privacy and access office (please feel free to call us for directions).
Processing fees
During the processing of requests there are certain fees are legislated. A fee estimate will be provided when the fees are expected to exceed $25. The hospital may require the requester to pay 50% of the total estimated fee prior to the work being effected if the estimate exceeds $100.The requester will receive a refund if the fee deposit paid exceeds the actual cost incurred by the hospital. | Manually searching a record | $7.50 for each 15 minutes spent by any person |
| Preparing a record for disclosure, including severing a part of the record | $7.50 for each 15 minutes spent by any person |
| Developing a computer program or other method of producing a record from a machine readable record | $15.00 for each 15 minutes spent by any person |
| Costs, including computer costs, incurred in locating, retrieving, processing and copying the record(s) if those costs are specified in an invoice received by the hospital | Actual costs incurred |
How to make a complaint under FIPPA
