Grand River Hospital (GRH) is not a single building. We operate our two primary sites in Kitchener, along with nearby buildings for specific services and satellite clinics with other hospitals in the Waterloo Region and Wellington County.

Our main locations are as follows:

Freeport Site: 3570 King Street East, Kitchener: GRH’s location for complex continuing care, rehabilitation programs, several outpatient clinics, satellite renal services, specialized mental health, and more.

Kitchener-Waterloo Site: 835 King Street West, Kitchener: GRH’s location for cancer care, critical care, emergency services, laboratory medicine, maternal-child programs, medical imaging, medical programs, inpatient mental health, paediatric care, pharmacy services, renal services and surgery.

850 King Street West: across King Street from the KW Site, this location offers a number of outpatient mental health services.

Hazelglen Outreach Mental Health Service, 236 Victoria Street North, Suite 1, Kitchener: Hazelglen helps people with mental health problems within the community, instead of the hospital. Hazelglen can be involved quickly to provide intensive, short-term mental health treatment services to help individuals and families get through a crisis. Referrals to Hazelglen are made by a care team within the mental health and addictions program.

Grand River Adult Withdrawal Management Centre, 52 Glasgow Street, Kitchener: GRH’s location for adult withdrawal management services and related programs.

Satellite locations: GRH works with several hospitals in the Waterloo Region and Wellington County to provide health care services, primarily with our renal and cancer care programs.
