What is personal health information?
Personal health information is any information in any form, electronic, written, verbal, etc. about an identifiable person. This includes information that is specifically health related, such as a person's medical condition or prescription medications, as well as information which is not always considered directly related to a person's health, such as his or her name, address, telephone number, or health insurance number. It also includes genetic information and blood and tissue samples.
Can I view my personal health information?

How do I get a copy of my health records?
If you were a patient of GRH, you may receive a copy of your health records by submitting a written request to GRH. You may download the Patient Authorization for Release of Health Record form in PDF format or obtain a copy of the authorization form by visiting our health information management department.
