St. Mary’s General Hospital and Grand River Hospital work together to develop and offer Digital Health tools such as:

· My Connected Care Patient Portal

·Pocket Health

· Virtual Care Video Appointments using WebEx

· Ontario Telemedicine Network (OTN)

There are several benefits of using Digital Health tools including decreasing time and travel costs, increased accessibility for patients with mobility difficulties, and using technologies that make it easier for caregivers to participate. These tools are especially helpful for those living in rural or remote communities.
