  1. Be involved in your care. Please ask a member of your health care team if you have questions. Please report any patient safety concerns or incidents to a member of your care team immediately.
  2. Please share important health information with your care team (health history, medications, allergies, etc.).
  3. Please make sure you know what to do when you leave the hospital.
